Caregiving: Tuesday, March 5, 2013
Caregiving is essential for the patient, because it makes them feel comfortable. Thus, it speeds up the patient's recovery. Caregiving includes the proper way of serving meals. Thus, the patient will have a proper diet and a healthy lifestyle. Caregiving is also considered as a "cheer up" for the patient, which makes him/her feel that he/she is still enjoying his/her life. It is also needed by the patient to have him/her a good care and someone that he/she can count on at all times. Even a little thing you will do, it's still important for them for you still did all the best you can to give care to the patient. I can say caregiving is vital because it helps the patient in terms of treatment as well as the whole being of the patient such as the physical, emotional, mental, spiritual and in all other aspects for the sake of the patient, and for the betterment of the patient's health.

- What are signs and symptoms?
In medicine a symptom is generally subjective while a sign is objective. Any objective evidence of a disease, such as blood in the stool, a skin rash, is a sign - it can be recognized by the doctor, nurse, family members and the patient. However, stomachache, lower-back pain, fatigue, for example, can only be detected or sensed by the patient - others only know about it if the patient tells them.

To sum it up, signs are what a doctor sees and symptoms are what the patient experiences.
2. When do we use a medical device such as bandaging?
Basically, we use bandage to prevent loss of blood if someone's having a wound. This may also be used for binding up fractures and other injuries and to hold dressing in place, to apply pressure or to immobilize a wound.
3. Where do bandages applied?
Bandages are usually used in some injured parts of the body. (e.g. leg, finger, arm, hand)
4. Why is there a need for bandaging injured parts of the body?
So that we may control the bleed (because excessive loss of blood can cause unconsciousness or even death) and to support a fracture or sprain/strain.
5. How do we apply bandage?
First is, we hold the bandage with the roll uppermost and start to roll around the wound upwards. For every turn, use a firm even pressure.
Then with each turn, we cover two-thirds of the width of the previous one. Avoid covering the tips of the fingers and toes.
And lastly, we fasten the bandage with a small safety pin or a bandage clip.

MAKING THE BED
1. Why do we need to know how to make an unoccupied and occupied bed?
2. How does making a bed help in the fast recovery of our patient?
3. As a good home nurse, how will you be able to render your services to your patient through bed making?
BATHING THE PATIENT
1. Why do you need to give a bath to the patient?
2. What are the materials needed for bed bath or sponge bath?
The materials to be prepared are as follows:
3. What’s the importance of using a urinal or bedpan?
4. What do you mean by back rub?
5. How do you differentiate cold compress from hot compress?
1. Why do we need to know how to make an unoccupied and occupied bed?
It is important to know how to make an occupied and unoccupied bed because if I were a home nurse, I should learn how to apply various home techniques such as these to make the patient feel comfortable and speed up the recovery.
Because if you do it in a proper way, the patient will feel comfortable when he's/she's in bed and to have him/her a better clean surrounding to avoid bacteria that might make the patient feel worse.
3. As a good home nurse, how will you be able to render your services to your patient through bed making?
I will do the bed making in an order and proper way and maintain a cheerful disposition so that the patient will feel comfortable and somehow will make him/her smile.
BATHING THE PATIENT
1. Why do you need to give a bath to the patient?
Because of course all of us people take a bath and it helps to clean the skin and keep it free from dirt, germs and sweat. It also helps the blood flow in the body. Baths give us chance to observe the patient and his/her skin. Baths are also very comfortable and relaxing to the patient
2. What are the materials needed for bed bath or sponge bath?
The materials to be prepared are as follows:
- 2 big towels
- 2 small towels
- a basin with warm water
- soap
- rubber sheet, and;
- light blanket
3. What’s the importance of using a urinal or bedpan?
Urinal and bedpan are important if the patient is expected to remain bedridden for a long period of time, so that the patient will not anymore get off the bed and enter the comfort room.
The back rub is usually administered after the patient's bath . It should be offered to the patient because it promotes relaxation, relieves muscular tension, and helps to stimulate our body circulation.
Cold Compress is an absorbent cloth that is being dipped in to a cold water, wrung out and applied to a body part to relieve pain or reduce inflammation or used as a comfort measure.
Hot Compress is the application of heat to a small area with hot gauze or hot compress cloth. Hot compress increases circulation locally. It relieves pain and congestion.
Home Nursing Techniques; Signs & Symptoms of Illnesses & Diseases: Friday, February 15, 2013
1.
If you are a good home nurse, what are the various home nursing
techniques that you need to apply that will make the patient feel
comfortable and speed up the recovery?
2. Why should the home nurse monitor the vital signs of the patient?
Of course, because recording patient observations is an important part of nursing routine and an essential skill to be able to detect changes in the patient's condition. And nurses should also feel empowered and confident enough to be able to adapt their routine to reflect a patient's progression towards recovery.
3. How do you take the body temperature, pulse rate, respiratory rate and blood pressure of the patient?
4. What are some practices that the home nurse must follow that will promote comfort to the patient?
5. How will you differentiate signs from symptoms of illnesses and diseases?
6. If you are the doctor, what will be your basis in giving the diagnosis for your patient?
To make the patient feel comfortable, as what I have learned in our TLE subject, we should keep the patient's room clean and times for a relaxed atmosphere, proper hygiene should be observed at all times to avoid infection and to also ensure speedy recovery of the patient and assure the patient of full support and understanding by maintaining a cheerful disposition.

3. How do you take the body temperature, pulse rate, respiratory rate and blood pressure of the patient?
In taking the patient's body temperature, we use thermometer. Thermometer can be either placed in the patient's mouth, armpit or in rectum. But usually nurses place it in the armpit.
In taking the patient's pulse rate, We locate the pulse by using either of our two fingers except the thumb (because the thumb has its own pulse and we might count it by mistake), then we can feel it either at the wrist, neck, ankle, or at the temple. Afterwards, we count the beats for 1 minute.
In taking the patient's blood pressure, we measure it by using a sphygmomanometer with the stethoscope to hear the signals for the systolic and diastolic pressure.
4. What are some practices that the home nurse must follow that will promote comfort to the patient?
The home nurse should practice the basic principles, characteristics, and duties and responsibilities that a usual home nurse must apply.
5. How will you differentiate signs from symptoms of illnesses and diseases?
A symptom is subjective from the patient point of view. A symptom is what the patient experiences about the illness, injury or disease. Symptoms can only be experienced, they are not able to be observed or measured objectively. Pain is a symptom. I do not know you are having pain unless you tell me. Nausea is also a symptom, as are: chills, numbness, fatigue, vertigo, malaise, itching, stomach cramps, burning on urination, etc. Symptoms can be characterized by the patient to help the physician diagnose the problem with that added information to consider along with the observations, measurements, laboratory findings and other diagnostic test results that are the objective signs.
A sign is an objective physical manifestation of illness, injury or disease. It is an objective finding, something one can observe and measure. A rapid pulse, a high temperature, a low blood pressure, an open wound, bruising, etc. are all signs. Signs give a more definite indication of the presence of a particular disease to the physician. Signs may be described as the current 'showcase' that the doctor observes in the patient and in the diagnostic testing and measurements.

6. If you are the doctor, what will be your basis in giving the diagnosis for your patient?
If I had given the chance to be a doctor, my basis would be a good communication between me and the patient and tell her/his story so that I would we would understand each other especially his/her situation in having a disease/sickness
Administering of Medicines to the Patient: Tuesday, February 12, 2013
I know most of my classmates including me are being such an ignorant when it comes to administering of medicines and such. Good thing TLE brainwashed us and invaded our brains to the of Health Care. When I was sick back then, medicines usually make me feel queasy, like you know, I don't like the taste of it, even though it's sweet. Whenever I have headache, stomachache, cough, sneeze and whatnot, I just usually rummage my dad's drawer which is full of medicines and get and take them. The problem is, I don't know whether the medicine will really alleviate my pain.
What I've learned was that patients have often been taking medications at home quite successfully for some time prior to admission to the hospital. So at the hospital, physicians and nurses often take this responsibility away from them, only to give it back again at discharge. To ensure the patient's safety, patients in their own care by having them administer their own medications while in the hospital. This is an excellent means to verify patient's understanding of their medications and provide education about the medications and how to administer them. Also, usually patients have the most at stake in making sure that their medications are administered correctly and on time, so they are likely to try to do it correctly.
Another benefit is that because they only have their own medications to administer, they may have a better chance of doing so on time than busy nursing staff might. The administration of a medicine is a common yet important for us. It's the most common manner in which a medicine is administered that will determine whether or not the patient gains any clinical benefit, and whether they suffer any adverse effect from their medicines. (As what I have researched) For example, intravenous (IV) furosemide administered too quickly can cause deafness; oral penicillin V given with food will not be well absorbed; over-application of topical steroids will cause thinning of the skin and may lead to systemic side-effects.

Our group made 4 recycled products and here they are:
Actually, our job was divided in to two groups. And I worked with the flower vases and necklaces.
As you can see, the first photo is called a birdie bank. You can save your money by just inserting your coins or even bills on the hole at the top of it. It's made of illustration board and I painted it with red and orange. And the bird! It's wings are made up of Popsicle sticks and the head was built up of carton with its googly eyes.
On the second photo are the necklaces which are made up of bottle caps from the soft drink bottles that we used to make flower vases. I just inserted some beads that were from Giro's old stuffs in the nylon string then glued them below the caps.
Then we made the flower vases. First we mached them with recycled bond papers and after a while we waited until it dried then painted the the whole two bottles with white and the other one was black. My work was on the middle of the third photo then Gabs continued it by painting it with dark blue on its body. It was just simple to do it but never did I notice, we finished them the whole afternoon. It was a bit exhausting but I guess it's still worth it.
And just for a small addition to our product, my group mates made a bracelet which we also inserted some small beads and some shells in the nylon string. And this was the simplest product that we did.
After doing all these products, of course, we felt a huge relief. As in thank God! Natapos na ang lahat :) I so much appreciated our work though it doesn't seems nice for some. And I really felt I saved our Mother Nature wala sa oras.
